Employment and Assistance Act and Employment and Assistance for Persons with Disabilities Act

Forms prescribed
1 Effective September 30, 2002,
(a) the attached forms are prescribed for the purposes of the Employment and Assistance Act and the Employment and Assistance for Persons with Disabilities Act:
(i) Enquiry;
(ii) Application;
(iii) Monthly Report;
(iv) Review, and
(b) the attached Application for Income Assistance for Child in the Home of a Relative is prescribed for the purposes of the Employment and Assistance Act.
