Venereal Disease Act Regulation
Notice to infected person
1 For the purposes of section 5 (1) of the Venereal Disease Act, the notice which may be given by a medical health officer to a person who is or may be infected with venereal disease or who has been or may have been exposed to infection shall be in the form set out in Form A.
Directions to infected persons
2 For the purposes of section 5 (2) of the Venereal Disease Act, the directions which may be given to an infected person by a medical health officer shall be in the form set out in Form B.

(Medical Health Officer) within ......... days, certifying that .................................... Directions to an Infected Person as to the Course of Conduct to be Pursued

As it appears that you are infected with venereal disease, I hereby direct you under the authority of section 5 (2) and (3) of the Venereal Disease Act
(a) to conduct yourself in a manner so as not to expose other persons to the infection,
(b) to take and to continue to take adequate treatment for the infection, and
(c) upon request, to produce evidence satisfactory to me that you are undergoing adequate treatment and in all respects complying with these directions.
If you fail to comply with these directions, action will be taken against you under section 6 of the Venereal Disease Act. (A copy of section 6 is attached.)
