I, Stan Hagen, Minister of Human Resources, order that, effective April 19, 2004, the Forms Regulation, B.C. Reg. 264/2002, is amended in page 4 of the Application Form and page 4 of the Review Form, under the heading CONSENTS, by adding "(Birth Registrations; Birth, Marriage and Death Certificates)" to the item for "Vital Statistics" at the end of the line, and by adding the following item after the item for "Vital Statistics":

· Department of Indian and Northern Affairs Development (DIAND);
