I, Michelle Stilwell, Minister of Social Development and Social Innovation, order that, effective September 1, 2015, the Forms Regulation, B.C. Reg. 95/2012, is repealed and the following regulation is made:

1  The attached monthly report form is prescribed for the purposes of the Employment and Assistance Act and the Employment and Assistance for Persons with Disabilities Act.
